Abstract
These following collection contains documents such as a Community Health Worker Program Overview, a Workplace Learning Overview, an Internship Requirement form, an engagement contract and an Attestation form. The purpose of the Program and Workplace Learning Overviews is to provide the student with a description of the role of the Community Health Worker. As part of the documentation provided to students CHW Internship Requirements form was provided stating the requirements for eligibility to be able to participate on the Internship at the end of the course work. To be able to record and made sure students had a complete understanding about their commitment and responsibility towards the program completion a student engagement contract was signed. To follow an effective and efficient protocol of communication, an Attestation form was created for students to communicate any changes in availability during the Workplace Learning Experience.
